The above figures are calculated for a couple with up to 2 children. The family will have 16 visits to share among them as desired. Explanation of the superscripts in the chart:
A. Visit fees for a non-member: 1 hour = $450 and 30 min = $225 and 15 min = $120.
B. A complete physical exam (CPE), is what you are used to getting with us now
C. A comprehensive functional assessment (CFA), is a 2-hour physical exam and you may email me for details.
D. Off of non-member fees, so a Gold member on a 30-min 5th visit of the year (first 4 visits are included in the Gold plan) would pay $112.50 while a Platinum member on a 15-min 11th visit of the year (first 10 are included in the Platinum plan) would pay $6
E. Our Electronic Health Record = EHR has a password-protected and secure patient portal where you can review your labs, health history and medication list or simply use it to communicate with the Medicatrix staff. If need be, you may print or share your health information while travelling in Texas or Colorado with a doctor seeing you for an emergency. Better yet, if that were to happen you can get me on the phone to discuss your care with that physician so nothing falls through the cracks.
F. This means Platinum members can access me 24/7 all year long, unless I am under general anesthesia. Now, that’s a great feature. Everyone else will be contacted by staff with advice and answer to any issues, of course under my supervision.
G. House Calls are made when necessary and feasible. The necessity will be medically determined by me and feasible simply means that you and I are within a reasonable proximity of one another. I am covering only Stanly County in full, and Montgomery plus Cabarrus Counties, if within 20 miles of my office. (The address is 500 N Fourth Street, Albemarle, NC 28001; you can Google map it for your own address and figure out your distance.) One obstacle to house calls would arise when either you or I are on vacation. We will work around that by phone, FaceTime or Skype.
H. “Cost” refers to supplies or medications used on rare occasions i.e. a brace to stabilize a fracture until Monday when an orthopedist can see you or an intramuscular (IM) antibiotic injection to prevent infection in a dirty wound to avoid a lengthy ER visit. A very small “cost” is also associated with in-house tests such as a strep test or urinalysis, which cost only a few dollars. These items would be charged to members at cost or with a small markup depending on tier of service. “Cost” does not refer to band aids, needles, syringes etc, which would be provided by Medicatrix.
I. Hospital, nursing home/ rehab care coordination and accompanying to specialist noted costs are for Stanly County only. Costs for other hospitals and nursing homes nearby are slightly higher but the service is available as it becomes necessary. Fees are available upon request and vary based on distance. Going to a specialist has to get coordinated with Medicatrix staff so that it would not interfere with care of other patients.
