Please read the following information carefully. When you call for an appointment, all required forms and the level of past history needed will be discussed with you. All forms are available on our website.
MEDICAL RECORDS
Medical records can only be released with your authorization. You are responsible for obtaining previous medical records from other physicians or health care providers. We will instruct you as to what records are necessary prior to each visit. Please contact your physician or other health care provider to obtain these records. Your records should be mailed to:

Once again, we would usually allow plenty of time for you to obtain these but it would help to check back with your doctor’s office within 72 hours of the original request to make sure this has been done. We will let you know the extent of past history needed.
CONSULTATIONS
If you are coming to us for a functional medicine consultation, please expect your initial visit to consist of a 60-minute medical assessment by Dr. Koohestani and a 60-minute visit with our nutrition expert, immediately thereafter. Allowing for time to check in and out, and to obtain your supplements, you should allocate about 3 ½ hours for this visit, so please plan accordingly. All the information gathered from your new patient questionnaire, old records, personal history, the face to face evaluations and diagnostic studies are used to uncover the root causes of your health concerns and thereby to design your personal health optimization program. Nutritional supplements are often recommended and we will help you select and find the highest quality products.
LABS
Cost of laboratory tests are variable and based on your medical needs. Conventional labs are ordered through national companies such as LabCorp or Quest Diagnostics. Please check your insurance coverage especially for an out-of-network ordering physician. If your primary care physician (PCP) feels comfortable, they can order these labs for you based on our recommendations. We would be glad to work closely with your PCP. We also order many advanced labs, the coverage for which is not necessarily very ideal through insurance carriers. These labs are ordered with utmost sensitivity as to their cost and only when our providers strongly feel that they would directly contribute to your overall assessment and treatment strategy. Once you have granted us permission for doing such tests, payments are due in full and would be charged to your account. Expect to learn about your lab tests within 7 business days of the results being reported back to us. Labs would require a face to face visit or phone encounter if their results prompt us to change your treatment plan in any form. Expect to have a visit with Dr. Koohestani after your lab results become available.
FEES FOR SOME COMMON SERVICES
Initial 60-minute Functional Medicine consultation with Dr. Koohestani: $450
Initial 60-minute nutrition consultation: $145
Follow up 30-minute office visits or telemedicine consultations with Dr. Koohestani: $250
Follow up 45-minute nutrition visits or telemedicine consultation: $100 (2 follow ups are sufficient for most new patients)
Follow up 15-minute office visits or telemedicine consultation with Dr. Koohestani: $125
Follow up 15-minute nutrition visit or telemedicine consultation: $40
Follow up 6-10 minute office visits or telemedicine consultation with Dr. Koohestani: $60-85
Follow up 5 minute nutrition telemedicine consultation, intended only to answer simple questions: $20
Text Support throughout 4-6 weeks of dietary changes: $60 (Mandatory for all patients going through dietary consultation)
Bioidentical hormone replacement therapy (BHRT) every 3 months: $350 or $1200/year after the initial assessment
Osteopathic manipulative therapy (OMT) 20-minute session: $125. This is for established patients who have had prior musculoskeletal assessment.
Lifestyle coaching/counseling 30-minute group sessions: $50, once a week (minimum commitment required 4 sessions), or a 60-minute private sessions: $150, once a week, with minimum 4-session commitment.
Second opinion, a special service offered at Medicatrix in collaboration with a team of board certified specialists, each an expert in their field of practice, carefully selected based on the nature of your medical dilemma. Fees are variable, starting at $2500. The second opinion is rendered only after a comprehensive review of your existing medical records, medications, supplements and treatment plans by our team at Medicatrix, the selected team of physicians, and a doctor of pharmacy. This requires a visit at Medicatrix with Dr. Koohestani and other members of this team as deemed necessary.
Please keep in mind telemedicine consultations are not available for patients who have never been seen at Medicatrix. These are only to accommodate our existing patients who live out of state or abroad in reducing travel costs. These appointments are charged the same way as a face to face visit. Telemedicine includes: texting, e-mails, phone calls, Skype or Facetime.
NUTRITIONAL SUPPLEMENTS
All the supplements recommended at Medicatrix are available for purchase at the office or at Health Nutz Natural Food Store located 5 minutes from us. Arrangements may be made for supplements to be mailed to you as well. These supplements are made available to you for convenience and you are not under any obligation to purchase them from us or Health Nutz. Please keep in mind that back orders are likely to occur when obtaining high quality supplements. We recommend that you prepare accordingly and order your supplements in advance so your treatment goes uninterrupted.
PAYMENT OPTIONS
At Medicatrix, your health and wellness are our first priority. Our medical decision-making is only influenced by the desire to provide you with access to the best diagnostic and treatment options available so that you are not limited to what is only covered by your health insurance policy. We ask for payment at the time of your visit. We accept cash, checks, or credit cards. We will also assist yout o use your Health Savings Account or Flex Spending Account (Please check with yoru employer on specific policies and covered services) All bank card transactions will carry a 3% charge.
WHAT INSURANCE DO WE ACCEPT?
We work with all of the health insurance carries as a non-participating provider. We will gladly provide you with the necessary documents including pertinent provider information, your diagnosis and treatment codes to submit to your insurance company for reimbursement. Since all insurance plans are different, we encourage you to contact your carrier before submitting your claim to ensure that you are using the correct claim form. Please keep in mind that some of the services we provide are not covered by insurance. Medicare and Medicaid do not reimburse for our services. We do not bill either, nor would we provide you with a bill to submit to Medicare or Medicaid.
HOW DOES REIMBURSEMENT WORK?
You will pay for your evaluation and treatment at the time of service. Our staff will prepare any necessary forms and provide you with the documentation and billing to submit to your health insurance provider for reimbursement. The insurance company will send a reimbursement check directly to you within 1-3 weeks. The amount of time may vary from one insurance carrier to another, but claims submitted by policy holders are usually given priority.
CANCELLATION POLICY
Should you decide to cancel your appointment, please allow at least 3 business days advanced notice to avoid being charged for the visit.

PRESCRIPTION & NUTRITIONAL SUPPLEMENT REFILL REQUESTS
It may take up to 72 hours to process a prescription refill request and even longer for some supplement. Please plan ahead to avoid any interruptions in your treatment plan
