Policies

1. Medicatrix Direct Primary Care (DPC) is a division of Medicatrix PLLC. The term member is referred to those who pay the Medicatrix membership fees on a monthly, quarterly or annual basis. Nonmembers are those who utilize Medicatrix, the practice, and its services on a pay-as-you-go basis and therefore do not have any membership rights to the Medicatrix DPC and its provisions. Services refer to all that Dr. Koohestani and his staff or independent contractors are permitted to perform under the laws of the state of North Carolina and which are consistent with their training and experience. Any treatment or medical services which Dr. Koohestani or his staff or independent contractors deem to be unfeasible, inappropriate or outside of their areas of expertise would not be offered as part of the membership.

2. Membership may be terminated with a 30-day written notice from you at any time and without explanation. If you have paid ahead, your money will be returned within a week. Medicatrix may terminate your membership with a written 30-day notice during which time you will have opportunity to find another physician. Any prepaid fees for the months for which your membership is terminated will be returned to you on your final membership day. If Dr. Koohestani plans to leave the community or close the practice, you will receive at least a 60 day notice. In case of Dr. Koohestani’s death, you shall get back any prepaid membership fees, for months not receiving services.

3. You may switch between tiers of service with a 30 day written notice.

4. Medicatrix DPC shall only include a maximum 700 active patients at any given time. This would translate into 200 Platinum Members, 300 Gold Members (see the accompanying chart for a description of these levels), and 200 Scholarship Members.

5. Scholarship Members are selected based on demonstration of financial need and upon completion and acceptance of the scholarship application. This application must be renewed annually and is subject to verification and refusal of acceptance upon change in an applicant’s financial status.

6. Medicatrix PLLC, Dr. Koohestani, the membership and the Practice do not participate with any health insurance. We do not bill your insurance company but can provide you with a detailed bill describing services rendered, which you may in turn submit to your insurance carrier for compensation, unless you are a Medicare or Medicaid recipient. Dr. Koohestani has opted out of Medicare.

7. Medicare recipients may not submit a bill to Medicare for our services and must annually sign an agreement attesting to such. Most of the services which make your membership at the Medicatrix DPC valuable and rewarding are not considered customary and acceptable services by Medicare and are not generally billable anyway. Medicare patients can, however, use their card for all that is ordered by our office including but not limited to imaging studies, hospital stays, referrals to specialists and labs.

8. Medicatrix membership is neither a health insurance plan nor an health maintenance organization, HMO. We recommend you maintain a health insurance plan, if you already have one, in order to meet your needs for services not rendered by Medicatrix membership such as hospital coverage, medications and specialist visits etc.

9. Although Medicatrix PLLC (Dr. Koohestani) does not accept insurance, you may still use your insurance for things we order such as X-rays, medications, covered labs and visits to specialists. Please check with your insurance carrier to see how things are covered when your physician is out of network. Dr. Koohestani will be considered out of network for all insurances.

10. Members will receive all in house tests such as strep test or urinalysis at cost or with a small markup depending on tier of service. LabCorp will pick up from Medicatrix so more expensive labs would get charged to your insurance and of course there are no fees for the blood draw for the members.

11. Small office surgeries such as removal of a skin tag or incision and drainage of an abscess and certain procedures such as ear wax removal have low but separate prices. You may already see a specialist for some of these, which would be fine but our discounted member prices may in some cases be lower than your specialist co pay for those who are insured and a steal for those who are not. This price list is available upon request.

12. The terms of the plan and our agreement may change with a 30 day written notice to that effect. You have the right to terminate your membership if you do not agree to the new terms or at any point without cause with a 30-day written notice.
